Following the requests of donators, BOD of Health Protection Foundation in Dąbrowa Górnicza beginning with 1^st February 2016 changes the refund rules of:

medicines, medical examinations difficult to access and rehabilitation services 

1. Refund of cost of medicines  purchased at w „Apteka Główna z Sercem”, which has been established in the place of „Apteka z Sercem” belonging to Health Protection Foundation, for employees that support the foundation and their family members(i.e. spouse or children at school age who are supported by parents) as well as pensioners and retirees supporting the foundation shall amount to:

50 % of purchase cost, at the amount exceeding PLN 50,00, not more than PLN2.000,00 /year.

2. Each time, an applicant shall be obliged to file a refund application at the foundation Patient Service Office prior to the medicine purchase, in order to show prescriptions or their copies on special occasions  and acquire the confirmation of fulfillment for the submitted application. Refund of medicines shall be performed based on submitted invoice in original within 14 days after the issuing of invoice.

 3. In particular cases, where acquiring the confirmation of the Patient Service Office for the refund is not possible, purchase of medicines in other pharmacies shall be allowed with the refund:

50 % of purchase cost, at the amount exceeding PLN 50,00.

4. When the instructions above are not observed – except for particular cases approved of by the Foundation BOD -  refund of medicines shall be reduced to:

30 % of purchase cost, at the amount exceeding PLN 50,00.

5. Refund of costs of medical examinations difficult to access  i.e MRI, CT, USG, Doppler echocardiography, OCT, Cardiac stress test and others required in treatment process (exclusive of medical examinations connected with  aesthetic medicine) shall amount to:

50 % of the service purchase cost, but not more than  PLN 1000,00 /year.

An applicant is required to submit the application with medical reference to the foundation Patient Service Office each time before the examination, and it shall be conducted after the acceptance granted by the office.

6. Refund of costs of rehabilitation  conducted by UNIMED  Sp. z o.o. Rehabilitation and Wellness Center in Dąbrowa Górnicza, al. J. Piłsudskiego 92 for a person entitled, supporting the foundation, as well as their family members, i.e. spouse or children at school age (aged 10-25 and supported by parents) as well as pensioners and retirees supporting the foundation shall be:

50 % of the service purchase cost, but not more than  PLN 1000,00 /year.
